Acute Care Team

The Acute Care Team is part of the multidisciplinary therapy service and helps with the rehabilitation needs of patients both in the site and on returning home.

The Acute Care Team is comprised of speech and language therapists, occupational therapists, physiotherapists, dietitians, multi-professional therapy assistant practitioners and therapy assistants. We form an integral part of the multidisciplinary teams, helping patients with their therapy needs in hospital and when discharged home.

The medical team cover all the medical and surgical wards on the site.

The wards covered by the team include:

  • AMU (respiratory patients) Ward 2
  • Ward 5 (Physiotherapy and Dietetic service only)
  • Ward 6 & 7
  • Critical care
  • Ward 11 & 12
  • Ward 14
  • Ward 15 A & B
  • Ward 18 & 19
  • Ward 21

There is a named therapist for each ward in each branch of therapy. The ward therapy teams work closely together to ensure patient care is seamless.

A neurology therapy team, who are based in the acute medical team, sees neurology patients. This team sees all neurology patients on-site to provide specialist neurology therapy care.

The nutrition team is also based within the acute medical team. It consists of gastroenterologists, gastro nurses, pharmacists and dietitians. It provides advice on patients who are referred for parenteral nutrition.

Booking

Referrals are received via "HISS," using a MPT screening tool to ensure referrals are appropriate.
