Children's Community Nursing

The aim of the service is to provide support to children with long-term or acute illnesses.

The team supports children in hospital who are acutely unwell. They assist these children in their transition back home and provide for their long-term ongoing health care needs after discharge. The aim is to reduce the length of stay and offer continuity of care for children and parents or carers at a difficult time. The team consists of eight nurses with specialist skills as well as paediatric training and a child will see one or two nurses depending on their needs.
